Lesões Condroblásticas Benignas • Matriz condrogênica:
o Aparência: lesão radiotransparente, margens irregulares e calcificações anulares, em forma de vírgula ou pontilhadas são em geral suficientes para estabelecer sua natureza condrogênica.
o No entanto, pode ser muito difícil determinar se um tumor cartilaginoso é benigno ou maligno.

Encondroma (Condroma) • O encondroma é o 2 tumor ósseo benigno
mais comum e representa o tumor mais comum dos ossos tubulares curtos da mão.
• Pode ter diferentes denominações a depender da sua localização: o Centralmente no osso: Encondroma o Extracortical: encondroma periosteal ou justacortical

Encondroma (Condroma)
• Os ossos tubulares curtos da mão (falanges e metacarpianos) são os locais mais freqüentes de ocorrência. • Costumam ser assintomáticos, de forma que muitas vezes são descobertos por uma fratura patológica.

Encondroma (Condroma) • Aparência:
o Nos ossos curtos, a lesão é totalmente radiotransparente, enquanto nos ossos longos pode exibir calcificações visíveis.
o Também podem ser reconhecidas por recorte das margens corticais internas, pois a cartilagem geralmente cresce em padrão lobular.

Encondroma (Condroma) • Aparência RM:
o Um encondroma é composto de vários lóbulos, que demonstram uma intensidade de sinal homogeneamente alta nas imagens em T2 ou STIR, geralmente, separados por septos finos com sinal de baixa intensidade.

Encondroma (Condroma) • Aparência RM:
o O padrão de anéis e arcos realçados é encontrado nos tumores cartilaginosos nas imagens pós-contraste, sendo encontrado tanto nos encondromas quanto nos condrossarcomas.
o Os achados de imagens, sugestivos de condrossarcoma e não de um encondroma benigno, incluem:
• Recortes profundos no endósteo (>2/3 do córtex) • Destruição cortical com ou sem uma massa de tecido mole
associada • Intensidade de sinal tipo edema na cavidade medular adjacente

Condroma Periosteal • Def.: Lesão de crescimento lento, que se
origina na superfície do osso dentro do periósteo ou abaixo dele.
• Geralmente há história de dor à palpação, freqüentemente acompanhada por edema, com localização preferencial na porção proximal do úmero.

Condroma Periosteal • À medida que cresce, erode a cortical do osso,

• O principal diagnóstico diferencial, particularmente em ossos longos, é o infarto ósseo medular.
• Uma tarefa difícil consiste em distinguir encondroma solitário de um condrossarcoma de baixo grau e crescimento lento.

Encondroma x Infarto Ósseo
• Por vezes, é bastante difícil distingui-las, pois ambas apresentam calcificações semelhantes.
• Os aspectos radiológicos úteis na diferenciação são: o Lobulação das margens corticais internas no
encondroma o Calcificações anulares, pontilhadas e em forma de
vírgula na matriz vistas no encondroma o Ausência de margem esclerótica que geralmente é
observada nos infartos ósseos.

Encondroma x Infarto Ósseo
• Fácil distinção pela RM. • Ao contrário dos lóbulos cartilaginosos que
formam o tumor condróide, um infarto medular é visualizado como uma região em forma de “chama” de intensidade de sinal heterogênea, freqüentemente contendo gordura, que é circundada por uma margem serpiginiosa de baixa intensidade de sinal em todas as seqüências.

Low-grade chondrosarcoma. A 48-year-old woman presented with pain in the upper leg. A radiograph shows a radiolucent lesion in the proximal tibia with a wide zone of transition and central calcifications. Note the thickening of the cortex, an important feature that distinguishes chondrosarcoma from similarly appearing enchondroma. On the excisional biopsy the lesion proved to be a low-grade chondrosarcoma

Encondroma - Complicações
• A complicação isolada mais importante é a sua transformação maligna em condrossarcoma.

Osteocondroma • Também conhecida como exostose

• É a lesão óssea benigna mais comum. • Faixa etária: geralmente antes da 3 década de
vida

Osteocondroma • O osteocondroma, que tem sua própria placa de
crescimento, geralmente pára de crescer na maturidade esquelética.
• Locais preferenciais: metáfises dos ossos longos.
• Apresentação: o Lesão pediculada, com um fino pedículo que se
afasta da placa de crescimento ou séssil, com uma base larga fixada na cortical.
o Geralmente apresenta calcificações na porção condroóssea do pedículo da lesão e capuz cartilaginoso.

• Faixa Etária: adolescentes e adultos jovens
• Localização: predileção por ossos dos MMII, principalmente porção distal do fêmur e proximal da tíbia.

Fibroma Condromixóide • Aparência Radiológica:
o Lesão radiotransparente excêntrica no osso, com uma margem recortada esclerótica, que freqüentemente causa erosão ou abaulamento da cortical.
o Freqüentemente, pode-se observar reação periosteal sólida.
o A lesão pode variar de 1-10 cm, com uma média de 3 a 4 cm.
