Acredito estar iniciando nova fase em relação ao exterior com a participação ativa em congressos internacionais, artigos em publicação e contatos para estabelecimento de convênios institucionais. Repercussão interna tem havido tendo em vista prêmios e honrarias recebidas, em função do trabalho realizado, entretanto, ainda há muita luta pela frente e certamente novos obstáculos!"

The study was conducted according to a factorial 2x2 cross-over design, performed in two phases of 14 days. The factors under evaluation were restorative material, at 2 levels: GIC- glass-ionomer cement and CR- composite resin; and age of restoration, at 2 levels: A- aged and N- not aged, resulting in 4 experimental groups: 1- GIC/A, 2- GIC/N, 3- CR/A and 4- CR/N.
Each group comprised 32 restored dentine slabs in duplicates or 16 experimental units (n = 16). They were randomly assigned to the 16 volunteers, which were considered as experimental blocks. In the first phase, eight volunteers wore appliances with specimens of groups 1 and 2 and eight with groups 3 and 4, avoiding a possible carry-across effect (Hujoel and DeRouen, 1992) provided by the GIC on the CR. In the second phase, volunteers that had worn appliances with specimens of groups 1 and 2, wore appliances loaded with specimens of groups 3 and 4 and vice-versa, characterizing the cross-over design.
The responses variables were fluoride concentration in the biofilm formed over the slabs ([F]: µµµµg F/g biofilm) and mineral loss (∆∆∆∆Z: vol % min x µµµµm). The null hypotheses tested were that there were no differences between the glass-ionomer cement and composite resin restorations, either aged or not, with regards to their cariostatic potential and concentration of fluoride in the biofilm on the specimens, when submitted to cariogenic conditions and fluoridated dentifrice exposure.

Gameiro GH, Nouer DF, Cenci MS, Cury JA. Enamel demineralization with two forms of archwire ligation investigated using an in situ caries model-a pilot study. Eur J Orthod. 2009 Apr 2. [Epub ahead of print] A modified in situ model to assess enamel demineralization around orthodontic devices was developed and a pilot study was conducted to evaluate two types of archwire ligation. Enamel blocks were placed in palatal removable appliances where orthodontic brackets were bonded. The brackets on one side of the appliance were ligated with elastomeric rings and those on the other side with stainless steel wires. Four volunteers (two males, two females), mean age 27 years, wore the appliances for 14 days during which time a 20 per cent sucrose solution was dripped eight times a day onto the enamel blocks. The biofilm formed around the brackets was collected for microbiological analyses and the mineral loss around the brackets was determined by cross-sectional microhardness measurement. The ligatures evaluated did not differ significantly from each other regarding biofilm weight, total bacteria, total streptococci, mutans streptococci, or lactobacilli counts (P > 0.05, Wilcoxon paired test). Enamel demineralization was also not different around the brackets for the different ligation methods (P > 0.05, split-split-plot analysis of variance). However, a statistical power analysis based on the data showed a trend to higher demineralization around brackets ligated with elastomeric rings. The developed modified in situ model may be suitable to assess the caries potential of clinical procedures used in orthodontic treatment.

Groups of 10 human enamel specimens were treated with one of: (a) amine fluoride (AmF), 1250 ppm F; (b) sodium monofluorophosphate (NaMFP), 1000 ppm F; (c) sodium fluoride (NaF), 1100 ppm F; (d) NaF, 250 ppm F; (e) Placebo (< 1 ppm F) dentifrices; or with aqueous solutions (f) NaF 900 ppm F or (g) NaF 30 ppm F. Lesions were assessed by cross-sectional microhardness. Mean +/- SEM DeltaZ (vol.% x microm) values of 3 dentifrices were: (a) 344 +/- 155, (b) 4259 +/- 257, and (c) 591 +/- 83. The AmF (1250 ppm F) was not statistically significantly different from the NaF (1100 ppm F) dentifrice in this model. Since the effect of calcium carbonate (CaCO(3)) based dentifrice on enamel demineralization is not clearly established, it was evaluated using the IEDT model described by Zero's group in 1992. This study had a crossover design and 10 volunteers were submitted to 3 treatment groups: a negative control, brushing without dentifrice; an active control, brushing with silica-based dentifrice (SiO(2)/MFP group), and the experimental group, brushing with CaCO(3)-based dentifrice (CaCO(3)/MFP). Both dentifrices contained 1,500 microgram F/g (w/w) as sodium monofluorophosphate (MFP). Enamel surface microhardness was determined in the dental blocks and the percentage change in relation to baseline was calculated. Fluoride uptake in enamel and its concentration in 'test plaque' were determined. The results showed that the dentifrice containing CaCO(3)/MFP was more effective than SiO(2)/MFP in reducing enamel demineralization (p < 0.05). A higher concentration of fluoride ion was found in 'test plaque' treated with CaCO(3)/MFP than in the negative control (p < 0.05). A low-fluoride (F) dentifrice has been recommended to reduce the risk of dental fluorosis, but its anti-caries efficacy is questionable compared with that of conventional dentrifices (1000-1100 ug F/g). The tested hypothesis was that conventional dentifrices might be safe if used soon after meals, since food interferes with F absorption. In a crossover, double-blind study, 11 volunteers ingested a dentifrice slurry containing 0 (placebo), 550 (low F), or 1100 microg F/g in 3 gastric content situations: on fasting, or 15 min after breakfast or lunch. F was analyzed in saliva and 24-hour urine samples. The conventional dentifrice ingested after lunch resulted in only 10% higher F absorption than the low-F ingested on fasting. Analysis of the data suggests that the risk of fluorosis could be reduced by the use of either a low-F dentifrice or a conventional dentifrice, if toothbrushing occurred soon after meals.

Although the anticaries effect of fluoride (F) dentifrices is clearly established, the relative importance of F taken up by dental plaque not removed by brushing and of F products (CaF2-like) formed on totally cleaned enamel for the subsequent inhibition of demineralization is not known. Both effects were evaluated using conventional (1,100 µg F/g) and low-F concentration (500 µg F/g) dentifrices in a randomized, crossover, double-blind in situ study. Enamel blocks not treated or pretreated with the dentifrices to form CaF2-like deposits were mounted in palatal appliances in contact with a Streptococcus mutans test plaque. Volunteers brushed with non-F (negative control), low-F or conventional dentifrices and inserted the appliance in the mouth. F concentration in the fluid and solid phases of the test plaque was determined after 30 min, and a rinse with 20% sucrose solution was performed. After additional 45 min, plaque was collected and the loss of surface hardness at different test-plaque depths was measured. CaF2-like deposition on enamel and F taken up by plaque due to the use of F dentifrices were able to significantly increase F concentration in the fluid phase of the test plaque, but only the latter significantly reduced the loss of hardness because of the 20–30 times higher F concentration. conventional dentifrices were observed for F on enamel, in plaque and on the subsequent loss of hardness. The results suggest that uptake of F by dental plaque not removed by brushing may be the main cause of the anticaries effect of F dentifrices.

The anticaries effect of professional fluoride (F) application has been attributed to calcium-fluoride-like deposits (CaF2) formed on enamel, but this has not been clearly demonstrated. We hypothesized that CaF2 formed on plaque-free enamel by F application would reduce enamel demineralization due to the increase of F availability in fluid of subsequently formed plaque. We created distinct levels of CaF2 on enamel to evaluate a dose-response effect. Enamel blocks were mounted in contact with a S. mutans test plaque and used in situ by 10 volunteers. F released to the fluid phase of this substrate ("plaque fluid") was measured before a cariogenic challenge. "Plaque fluid" F concentration was highly correlated to the enamel CaF2 concentration (r = 0.96, p < 0.001) and to consequent enamel demineralization (r = -0.75, p < 0.001). The results suggest that F released to plaque fluid from CaF2 formed on enamel may play a significant role in the anticaries effect of professionally applied F agents.

INTRODUÇÃOCárie dental é uma doença multifatorial e seu desenvolvimento está associado àpresença de microrganismos cariogênicos [Sansone et al., 1993 refe inapropriada para o afirmado ] e ao alto consumo de carboidratos [Axelsson, 1991 refe inapropriada]. A sacarose é considerada o mais cariogênico dos carboidratos [Newbrun, 1969] e tal característica é principalmente atribuída àalta energia proveniente de sua hidrólise, a qual pode ser utilizada pelas bactérias do biofilme dental para a síntese de polissacarídeos extracelulares [Hamada & Slade, 1980 não sei se é a melhor refe para o dito]. A presença de polissacarídeo extracelular aumenta a porosidade do biofilme dental [Dibdin & Shellis, 1988] e prolonga o decréscimo de pH [Zero et al., 1986] causado por ácidos provenientes do metabolismo bacteriano [Yamashita et al., 1993 refe inapropriada e desnecessária]. Conseqüentemente, uma alta freqüência de consumo ou exposição à sacarose leva a um maior número de episódios de decréscimos de pH no biofilme dental [van Houte, 1994 refe inapropriada e desnecessária ], que diminuem o tempo de remineralização pela saliva, favorecendo a desmineralização da superfície dental [Loesche, 1986 não sei se éa melhor refe para o dito ]. ......influência no desenvolvimento de lesões cariosas [Newbrun, 1982] e essa relação tem sido melhor estudada nos últimos anos [Cury et al., 1997, 2001; Duggal et al., 2001]. Em acréscimo, o aumento da freqüência de exposição àsacarose 20% altera as características bioquímicas deste biofilme, o qual apresenta maiores quantidades de polissacarídeos extracelulares e menores níveis de cálcio, fósforo e flúor [Cury et al., 1997]. Embora esteja bem definido o papel da freqüência de consumo de açúcar na composição do biofilme e no desenvolvimento de cárie dental, há evidências de que a concentração de sacarose em cada exposição possa também ser um fator relevante. Tehrani et al. [1983] verificaram, em um estudo in situ de curta duração, que dois bochechos com solução de sacarose 1% não resultaram em perda mineral. Porém, mantendo-se a freqüência e aumentando a concentração da solução para 5%,
observou-se desmineralização dental (Assim, vc tem que ter uma boa justificativa para ter feito este trabalho).....

Em acréscimo, uma relação direta entre concentração de sacarose e cárie foi verificada em animais [McDonald & Stookey, 1977; Hefti & Schmid, 1979 estas refe deveriam anteceder a de Tehrani, elaborando um parágrafo que deixe claro a relevancia do seu trabalho], assim como uma relação indireta entre concentração de sacarose e pH (use termo apropriado ao metabolismo fermentativo. INTRODUCTION OriginalSweeteners are considered effective substitutes for dietary sugars and, in recent years, their consumption has increased [[[[Imfeld, 1993]]]]. Evidence in the literature suggests that sweeteners such as saccharin and cyclamate [[[[Imfeld, 1993]]]], xylitol [[[[Bowen & Pearson, 1992]]]], sucralose [[[[Meyerowitz et al, 1996]]]] and aspartame [[[[Das S et al, 1997]]]] are not cariogenic per se. On the other hand, they are used in powder formulations together with excipient because of its physical and chemical properties. Maltodextrin is used, as an excipient, in powder formulations containing sucralose [[[[Meyerowitz et al, 1996; Grenby & Mistry, 2000]]]]. In Brazil, the excipient of choice is lactose due to its high solubility, low higroscopicity [[[[Jaiyeoba & Spring, 1980]]]] and reduced production cost. Sweeteners marketed as Zero Cal, Finn, Gold, Splenda, Stevita, among others, contain lactose in their composition, whose concentration in these products can achieve 99%...................

However, the commercial products contain several components and the formulation may not be totally safe for the teeth. For example, sugars or their derivatives have been added mainly to powdered sweeteners as bulking agents. In Brazil lactose is the excipient found in most powdered sweetenersLactose is fermented by oral bacteria, but the pH drop in dental plaque is lower than after sucrose intake [Birkhed et al., 1993], so that lactose may be considered as moderately cariogenic for enamel [Bowen et al., 1991].However, the critical pH for dentine is higher than for enamel [Hoppenbrouwers et al., 1987], implying that sweeteners containing lactose can be more cariogenic for dentine. In addition, the main target would be the adult population, who frequently consume table-top sweeteners in several small cups of coffee per day, especially in Brazil. Furthermore, root dentine exposure is common in adults.This study was therefore undertaken to evaluate the effect of lactose present in sweeteners on dentine demineralization.

INTRODUCTION OriginalThe cariogenicity of sucrose is well demonstrated in previous researches [Newbrun, 1982; Cury et al., 1997, 2000; Sheiham, 2001; Aires et al., 2006], which is partly attributed to its role as a substrate for the synthesis of extracellular polysaccharides (EPS) [Rolla et al., 1985]. EPS increases the porosity of dental biofilm [Dibdin and Shellis, 1988] and maintains a low pH value [Zero et al., 1986], which results in the enrichment of acidogenic and aciduric species, such as mutans streptococci and lactobacilli [Marsh, 2003]. Cury et al. [1997] showed that biochemical characteristics of biofilm are modified in presence of sucrose, with high concentration of polysaccharides and low concentrations of calcium, inorganic phosphorus and fluoride. Some studies [Pecharki et al., 2005; Ribeiro et al., 2005], analyzing a dental biofilm formed in situ, showed higher counts of mutans streptococci and lactobacilli in the presence of sucrose than in the negative control group. However, the biofilm analysed in those studies were formed during 14 or 28 days and it is important to evaluate the biofilm at shorter periods in order to observe its dynamics of maturation.Thus, the aim of this study was to evaluate the microbiological and biochemical composition of dental biofilm formed in situ in presence of sucrose or glucose + fructose during 3, 7 and 14 days and its relationship with enamel demineralization.
INTRODUCTION modificadaSucrose is a unique cariogenic carbohydrate, since it is fermented to acids and also metabolized into extracellular polysaccharides (EPS) by microorganisms of dental biofilm. The lower average pH induced by sucrose fermentation triggers a shift in the balance of resident plaque microflora to a more cariogenic one [Marsh, 2003]. Furthermore, this low pH is responsible for enamel-dentine demineralization, whose detection is time-dependent.In addition, insoluble EPS change the biofilm matrix, increasing its porosity [Dibdin and Shellis, 1988] and allowing sugar diffusion into the deepest parts of the biofilm, where low plaque pH can be reached [Zero et al., 1986]. Also, EPS may enhance bacterial adherence to the tooth surface [Schilling and Bowen, 1992], increasing plaque formation and accumulation. The relationship between EPS and enameldemineralization has been well supported by in situ studies [Cury et al., 2000, among others]. EPS are considered critical virulence factors in the dental biofilm formed in the presence of sucrose [Bowen, 2002], but simultaneously it has been found that dental biofilm exposed to sucrose presents reduced concentrations of calcium, inorganic phosphorus (Pi) and fluoride [Cury et al., 1997 among others], which are responsible for the solubility equilibrium of mineral tissue and their concentrations in plaque fluid determine the pH below which enamel or dentine will demineralize.
On the other hand, although a strong association between these changes induced by sucrose on biofilm composition and enamel demineralization has been found [Cury et al., 1997, 2000; Aires et al., 2006], the temporal relationship between the changes in the biofilm and enamel demineralization have not been established, since in previous work they have both been measured at the end of the experimental period. Therefore, this study aimed to evaluate the timing of the effects of sucrose on biochemical and microbiological composition of dental biofilm and its relationship with enamel demineralization.

INTRODUCTION OriginalThe evidence that sugars are involved in the development of dental caries has been shown in historical, epidemiological, clinical human research and experimental research in animals studies (Touger-Decker & van Loveren, 2003). Among the sugars, sucrose is considered the most cariogenic carbohydrate, because it is fermented by the microorganisms and is the substrate for the synthesis of extracellular polysacharides (EPS) in dental biofilm, allowing the penetration of acidogenic substrate into the internal layers of the biofilm and consequent acid production close to the dental structure (Newbrun, 1967, Dibdin & Shellis, 1988). In addition, high frequency of sucrose exposure can modify the biochemical composition of dental biofilm, which then presents high concentrations of insoluble polysaccharides and low concentrations of calcium, inorganic phosphorus and fluoride (Cury et al., 1997). Changes in inorganic concentrations can also happen in the biofilm fluid, which is important because the main ionic exchanges occur in the fluid between tooth surface and biofilm.……..Thus, controversy in literature still exists in relation to the development of the dental caries and its relation with frequency of carbohydrate consumption, considering the use of fluoride dentifrices. The aim of the present study was to evaluate, using an in situ model with a palatal intraoral device, the effect of different frequencies of sucrose exposure on microbiological and biochemical composition of dental biofilm formed in this conditions and enamel mineral loss.

The incidence of dental caries has declined in developed1 and in developing countries2, but the increase of other dental lesions, such as tooth wear has been observed.3 Dental wear has a multifactorial etiology, and wear scores result from the interaction of physical and chemical agents, although the effect of one factor – erosion – have been frequently dominant in the occurrence of the wear.4
The erosion is a loss of dental hard tissue due to chemical dissolution of teeth by acids of non-bacterial origin, produced because of endogens acids or extrinsic factors frequently occasioned by consume of acids foods and drinks, and by exposition of acids in the environment. In the modern society, the consumption of potentially erosive foodstuffs and drinks, e. g. fruits, salads, sport and soft drinks, is increasing for a variety of reasons including dietary recommendations by various especially health-minded groups.5-9 Usually, these groups of persons are well instructed in oral hygiene consequently, their risk of loosing tooth substance due to dental cleansing is high, since abrasion or attrition immediately after an acid attack is greater than without previous exposure of the teeth to erosive agents.4,10-12

Erosion is a chemical phenomenon of acid attack to dental hard tissues and the consequence for dental surface depends on the subsequent occurrence or not of a mechanical effect of abrasion. In the beginning of the process of erosion, the mineral of the dental surface are dissolved with softening of the surface (Featherstone and Lussi, 2006). The loss of structure by acid attack alone may be not a concern (Hooper et al, 2003) because the softened surface may be rehardened by the remineralizing action of saliva (Attin et al, 2001; Rios et al,2006 n19;). However, if the hard dental surface softened by acid is subjected to abrasion there will be an irreversible mechanical loss of dental structure (10;4; 41;) Abrasion forces are present in oral cavity either naturally by teeth, tongue and cheek attrition, and also they can be produced by toothbrush during brushing of the softened dental surface (11). Therefore, most the time acid erosion is not an isolated phenomenon since it is ever subsequently associated with some abrasive effect. Preventive measures to reduce either the effect of erosion or the effect of abrasion on acid softened dental hard tissues have been investigated…….Fluoride dentifrice has been evaluated as an alternative to reduce erosion……..

VERSÃO� Professional topical fluoride (F) application is considered an effective measure for caries prevention (Marinho et al., 2007)�The efficacy of topical fluoride treatment is related to the soluble fluoride availability in the commercial product, since to react with enamel/dentine, F in the product should be soluble.�Fluoride varnishes present F in a concentration higher than the solubility product of the fluoride salt added, and are prepared in an alcoholic base. Thus, the amount of soluble F should be reduced.

�The anticaries mechanism of professional F application is attributed to products of reaction (CaF2-like and Fluorapatite) formed on dental hard tissues during the application and retained over time (ten Cate, 1997).
�To form products on hard tissues, fluoride present during the application should be chemically available (soluble) to react with enamel-dentine (White & Nancollas, 1990)
�NaF, a soluble salt (4%), has been used in commercial products in alchoholic media, where it is fairly soluble. Furthermore, CaF2 has been also added in professional products for dental use. CaF2 salt is almost insoluble in water (0.015%) and ethanol is the solvent used in commercial products.
