infección por SARM de forma exclusiva como para la
producida por cualquier patógeno multirresistente, al ingreso
en UCI y basado exclusivamente en factores de riesgo clínicos

Major comments The authors should explain how colonization was determined at ICU admission. Were surveillance cultures obtained in all patients? Did you use selective media? What bacteria were searched?

...the other major observation that I have about your manuscript is that you do not describe how you identified patients with antribiotic-resistant bacteria - was there a systematic screening process in place, and if so, what did this consist of. I do appreciate that you have undertaken a large multi-centre study of an important subject area... if you were able to address these points in a new manuscript, it might well be suitable for publication in the JHI, and we would be very happy to receive submission of a new paper.
One of the main positive characteristics of the study is that it is based on a huge prospectively collected data set of close to 70,000 critically ill patients with a length-of-stay >24 h. in 147 Spanish ICUs, although the data base was not designed specifically for the purpose of this type of study.
Limitaciones
Reviewer #4: This is an interesting approach to establish the factors associated with colonization and/or infection by multi-drug resistant pathogen at admission to the ICU. This is a nationwide program of high quality that may allow the authors to obtain valuable conclusions. Validation of the identified variables in another cohort adds value to this research. The topic is of interest especially in the current situation of high incidence of infections caused by MDR pathogens. Moreover, in the literature, there is no abundant information on this matter. I have the following comments to improve manuscript quality.
