Exudative retinal detachment: case reportA 38-year-old woman developed a massive exudative retinal

detachment after receiving verteporfin [Visudyne].The woman presented with a peripheral retinal capillary

haemangioma (RCH) associated with von Hippel-Lindaudisease in the left eye. She underwent photodynamic therapy(PDT) with verteporfin [dosage and route not stated], with aspot size of 8000µm to the haemangioma for 83 seconds, followed by another laser spot to the feeding and drainingvessels of another 83 seconds. One day after PDT, shereported severe visual impairment in the treated eye; her best-corrected visual acuity was counting fingers. A B-scanechography and fundus examination confirmed an extensiveretinal detachment complicated by a vitreous haemorrhagethat obscured clear fundus visualisation.

Since the retinal detachment progressed, and visual acuitydeteriorated to hand motions 3 days after PDT, the womanunderwent a lens-sparing pars plana vitrectomy, with externaldrainage of subretinal fluid. The retina was successfullyreattached. One month later, her best-corrected visual acuityimproved to 20/100 in the left eye, with complete resolution ofthe serous retinal detachment at the macula. Her RCH wasstabilised without recurrences, and her visual acuity at lastfollow-up was 20/100.

Author comment: "[W]e describe for the first time a casethat required emergency surgery to treat a massive vision-threatening retinal detachment immediately after theapplication of PDT with verteporfin for peripheral RCHassociated with von Hippel-Lindau disease".Goto S, et al.
