Genotipos A y C comprometen el 35 y 31% de virus respectivamente en los EUA.

EPIDEMIOLOGIA Globalmente 350 millones de personas crónicamente
infectadas con VHB.
600,000 muertes anuales.
88% de la población mundial vive en regiones donde la prevalencia de infección por VHB crónica es mayor al 2%.
Prevalencia en EUA del 0.4%, con estimado de 0.8 a 1.4 millones de personas crónicamente infectadas.
Con la vacunación 1991, la incidencia a disminuido de 11.5 casos por 100,000 personas en 1985 a 1.6 casos por 100,000 personas en el 2006.

Inoculacion percutanea (16%).

TERAPIA ANTIVIRAL APROBADA PARA INFECCION VHB CRONICA EN PACIENTES QUIENES NO HAN RECIBIDO TX.
•Interferon α pegilado 180 ɥg/sem SC por 48 semanas
•Lamivudina 100mg/sem por 48 a 52 semanas
•Adenofovir 10mg/dia ≥48 semanas
•Entecavir 0.5mg/dia ≥ 48 semanas
•Telvibudina 600mg/dia ≥ 52 semanas
•Telmofovir 300mg/dia ≥ 48 semanas

EPIDEMIOLOGIA Globalmente 170 millones de personas
o 3% de la poblacion mundial son cronicamente infectadas con VHC.
En EUA el 2% de la poblacion adulta mayor a 20 años tiene anticuerpos contra VHC.
Prevalencia mayor (8-9%) en grupos de alto riesgo.

Disminuir progresion de fibrosis hepatica.
Prevenir desarrollo cirrosis.
Mayor periodo libre de sintomatologia.

PERSONAS QUIENES TX. ACEPTADO VHC CRONICA. Mayor a 18 años
Nivel serico anormal de ALT.
Biopsia hepatica con hepatitis cronica con significante fibrosis.
Enfermedad hepatica compenzada (BT <1.5 g/dL; INR <1.5; albumina >3.4 g/dL; plaquetas >75,000 k/mm; sin encefalopatia o ascitis.
Marcadores hematologicos aceptables (hemoglobina >13 g/dL para hombres y >12 g/dL para mujer; neutrofilos>1.5 k/mm; creatinina < 1.5 mg/dL)

INDIVIDUALIZAR TRATAMIENTO
ALT normal Tratamiento previo fallido (sin respuesta o
recaida). Usuarios drogas o alcohol. Leve o sin fibrosis en biopsia hepatica Hepatitis aguda. Coinfeccion with VIH Menores a 18 años IRC Cirrosis descompenzada.
