PATOLOGIA TUMORAL
Incomum (4% dos tumores do tracto GI), mas o tumor maligno mais frequente no duodeno.
++ esporádico; associação com PAF, Peutz-Jeghers, doença de Crohn.
++ na região peri-ampular; raro no bulbo.
Trânsito: lesão anular constritiva, massa ulcerada ou polipoide; imagem “em caroço de maça” é típica.
TC – espessamento parietal, com realce; tipicamente causa obstrução (≠ linfoma); adns perilesionais; metástases síncronas 5-40%.
DD com tumores malignos da ampola, pâncreas e VBP.

duodenum, causing deformity of the lumen

(arrowhead), rendering the lesion surgically
unresectable. There is also periduodenal fat
stranding and adenopathy (circle).

Linfoma
Linfoma do tracto GI é mais comum no ileo distal; duodeno é o local menos frequente.
Geralmente 2ário a doença ganglionar.
Substituição da camada muscular pelo tumor e destruição do plexo autonómico causa a característica dilatação aneurismática do
lumen (obstrução não acontece).
Trânsito: pregas espessadas, dilatação luminal, lumen normal.
TC: segmento longo de espessamento parietal marcado, circunferencial e assimétrico; de densidade relativamente homogénea e fraco
efeito de realce; dilatação aneurismática, massa exofítica, massa polipóide; grandes adns paraduodenais e retroperitoneais; +/-
esplenomegália.

Tumores periampulares são neoplasias benignas ou malignas com origem a menos de 2cm da ampola de Vater. Subdividem-se em; adenoCa da cabeça pancreática; colangiocarcinoma da VBP distal; adenoCa da 2ª porção do duodeno; verdadeiros tumores ampulares. A maioria é adenoCa.
2 tipos de verdadeiros tumores ampulares: adenoma (benigno); adenocarcinoma (maligno).
Devido às manifestações clínicas e imagiológicas semelhantes (obstrução biliar e do Wirsung), o DD entre os vários tipos é difícil.
CPRM pode ser útil na determinação da origem do tumor.
Trânsito: sem alterações ou “bulging papilla sign” (que pode estar presente em 30% dos trânsitos normais).
TC - espessamento parietal; massa polipóide com origem na parede medial da 2ª porção do duodeno; dilatação VBP e do Wirsung (sinal do duplo ducto).

Tumor da submucosa, constituído por adipócitos rodeados por uma cápsula fibrosa.
++ solitários; em qualquer parte do duodeno.
++ incidentalomas; alguns podem manifestar-se como o ponto de partida de invaginação intestinal.
Trânsito: defeito de repleção oval/esférico; bem definido, margens regulares; compressível à compressão fluoroscópica.
Diagnóstico definitivo com TC ou RM através da demonstração de densidade/ intensidade de sinal de gordura.
Complicações muito raras: ulceração ou invaginação - podem complicar o diagnóstico.

GIST – tumor do estroma gastro-intestinal – é o tumor benigno mais comum do intestino delgado.
Origem nas células intersticiais de Cajal; expressão do factor de crescimento tirosino-quinase (c-KIT – CD117).
Origem na camada muscular própria ou muscularis mucosa.
70-80% são benignos. Comportamento benigno mais frequente no estômago. No restante trato GI (incluindo duodeno) têm maior
probabilidade de ser malignos.
Trânsito: massa tipicamente exofítica; pode ser intramural ou intraluminal.
TC – lesão oval; realce “patchy” e heterogéneo; áreas centrais de necrose; podem sangrar; calcificação pouco comum. Tumores
pequenos raramente necrosam, podendo apresentar densidade homogénea.

Difícil diferenciação entre benigno e maligno com base nas características imagiológicas ou histológicas.
Histologicamente, grande número de mitoses prediz potencial maligno.
Geralmente, tumores >5cm têm maior probabilidade de serem malignos, mas não há características específicas em TC.
Necrose pode ocorrer tanto em tumores benignos como malignos.
O sinal mais específico em TC é a presença de metastases síncronas (fígado e peritoneu ++).
Uma vez que não há disseminação linfática, não há geralmente adenopatias.

Tumores raros, com origem em células neuroendócrinas do pâncreas ou similares fora do pâncreas.
Divididos em funcionantes e não funcionantes (conforme secretem ou não hormonas). ++ malignos (60-92%).
++ insulinomas e gastrinomas.
Insulinomas encontrados quase exclusivamente no pâncreas. Gastrinomas situados no triângulo do gastrinoma em 90% (área anatómica
que inclui cabeça do pâncreas e 1ª e 2ª porção do duodeno).
TC – nódulo pequeno em localização apropriada; múltiplo em 10%; hipervascular; realce maior que o pâncreas em fase arterial e
portal; podem ser confundidos com estruturas arteriais peripancreáticas.
Utilizar água como contraste para aumentar taxa de deteção; fazer fase arterial pura (mais sensível que fase parenquimatosa pancreática).

Histopathological study following
pancreatoduodenectomy revealed duodenal and
pancreatic neuroendocrine tumors (gastrinomas).
Fig 4 and 5 - Abdominal CT shows a mass (arrows) in
the medial wall of the 2nd portion of the duodenum.
The tumour displays intense contrast enhancement in
the arterial phase (Fig 4), with retention in the
venous phase (Fig 5) of the dynamic study.
The high spatial resolution provided by CT clearly
shows that the lesion is independent from the
pancreas.

Lesões metastáticas para o trato GI são mais frequentes que tumores primários.
Lesões duodenais são mais frequentes na região peri-ampular, seguida do bolbo.
Primários mais frequentes: mama, pulmão, outros tumores GI, melanoma.
Trânsito: origem na parede ou subserosa do duodeno, mas à medida que crescem tornam-se intraluminais, podendo mesmo ulcerar.
TC: inespecífica; suspeitar de metastases quando se encontrar qualquer espessamento nodular parietal do trato GI em doente com
neoplasia já conhecida.
Fig 1 and 2 – Abdominal and pelvic CT of
a patient with sigmoid colon cancer,
performed to stage the disease, clearly
demonstrated the sigmoid tumor
(arrowheads) but failed to show distant
metastases. However, MPR and
retrospective analysis revealed a discrete
circumferential thickening on the 3rd and
4th portions of the duodenum (arrows)
which was not recognised initially.

Ca gástricos podem estender-se através do piloro para o bolbo duodenal ou, de forma alternativa, invadir a 4ª porção duodenal.
A recidiva de Ca gástrico na anastomose de BII é uma das principais causas de sindrome da ansa aferente.
Ca do colon pode causar efeito de massa, invasão ou formação de fistula colo-duodenal.
Ca pancreático agressivo invade muitas vezes o duodeno, embora não seja critério de irressecabilidade.

neoplasm of the hepatic flexure of the colon. An
undefined mass apparently involves the duodenum
(asterisk) and the colon. A small amount of positive oral
contrast agent (arrowhead) is present within the mass.
An image obtained caudally (Fig 2) shows oral contrast
agent in the colon (arrowhead) without evidence of
contrast in the jejunum (arrow), a feature related to a
colo-duodenal fistula.

Pólipos duodenais são muito raros isoladamente. ++ múltiplos na sequência de síndromes polipóticos: PAF; Peutz-Jeghers, Gardner e
Polipose Juvenil.
DD de múltiplas lesões polipoides duodenais inclui: metástases; linfoma, hiperplasia nodular linfoide e sarcoma de Kaposi.
Trânsito: múltiplos defeitos de repleção com origem mucosa (sinal do chapéu de coco).
DD com pseudopólipos – defeitos de repleção na doença de Crohn e colite ulcerosa devidos a regeneração pós-inflamatória de tecido
não ulcerado.
Verdadeiro pólipo é redondo, bem definido, com bordos regulares; Pseudopólipo é filiforme ou com aspecto em Y, irregular e mal
definido, com inflamação adjacente.
TC: lesões polipoides endoluminais, que podem ou não realçar (na polipose juvenil os pólipos contêm mucina e não realçam).

Entidade muito rara – +/- 150 casos descritos.
Proliferação hamartomatosa das glândulas de Brunner, que se localizam maioritariamente no bolbo e duodeno proximal, na
submucosa.
Patogénese e anatomia patológica pouco conhecida; pode evoluir para adenoma e adenoCa.
Variáveis em tamanho de 1 a 3 cm. Invaginação pode acontecer.
Trânsito: pólipo séssil ou pedunculado (++), com origem submucosa, de aspecto inespecífico.
TC – lesão polipóide endoluminal inespecífica.
DD com adenomas, leiomiomas, tumores neuroendócrinos, tecido pancreático aberrante, mucosa pilórica prolapsada para o duodeno.

PATOLOGIA NÃO TUMORAL
Resultam do prolapso da mucosa ou de toda a parede duodenal. Podem ser múltiplos. Atingem qualquer parte do duodeno (++ parede medial 2ª porção).
++ Achados incidentais. Divertículos periampulares podem causar obstrução biliar e colangite.
Trânsito: imagem de adição sacular; DD com úlceras pela visualização de pregas mucosas no colo do divertículo e alteração morfológica com peristalse.
TC: saco com líquido e/ou gás em localização típica, com comunicação com o lumen duodenal.
Um divertículo completamente preenchido por líquido pode confundir-se com lesão quística do pancreas. Se tiver gás pode confundir-se com abcesso.

Malformações raras; resultado de forças propulsivas anterógradas crónicas sobre uma membrana endoluminal congénita incompleta. Portanto têm dupla camada mucosa.
Apesar de congénitos, manifestam-se em adultos de forma inespecífica ou como complicações (obstrução mecânica; hemorragia, colangite ou pancreatite).
Trânsito: Sinal da manga de vento é patognomónico – contraste dentro e fora do lumen do diverticulo, separado pela parede radiolucente do divertículo.
TC – coleção endoluminal com densidade de líquido dentro do duodeno; o calibre duodenal pode estar aumentado.
Fazer MPR pode ajudar a reveler o colo e sua inserção na parede duodenal.

Úlceras quase exclusivamente causadas por H. pylori.
95% ocorrem no bolbo; não são consideradas pré-malignas.
Trânsito: imagem de adição, ausência de padrão mucoso, colar edematoso (mucosa inflamada), pregas espessadas a convergir para o nicho ulceroso, sem modificação morfológica com a peristalse.
Complicações: hemorragia, perforação, estenose – situações em que a TC tem melhor acuidade.
Duodenite corresponde a inflamação duodenal sem formação de úlceras.
Principais causas: H. pylori, álcool, AINEs
Trânsito e TC: espessamento e realce parietal, nódulos (glândulas de Brunner hipertrofiadas), deformidade do bolbo, erosões.

Os quistos do colédoco são entidades raras que correspondem a dilatação da VBP. Diagnóstico feito quando há dilatação
disproporcional da VBP em relação às VBIH, após exclusão de causa mecânica (cálculos, tumores, inflamação).
O coledococelo corresponde à “herniação diverticular” da parte distal da VBP para dentro do lumen da 2ª porção do duodeno, sendo
o tipo mais raro (2%) da classificação de Todani (tipo III).
Alguns autores consideram-no uma variante de quisto de duplicação entérico.
Complicações: colecistite, colangite, estenoses biliares, coledocolitíase, pancreatite, malignização para colangiocarcinoma.
TC: saco preenchido por líquido que protrude para o lumen duodenal, mas não opacifica com contraste oral.
CPRM pode ajudar quando TC é duvidosa.

Os quistos de duplicação entéricos têm origem mais frequente no intestino delgado. No duodeno são mais frequentes em D2 e D3.
++ incidentalomas. Alguns podem conter tecido gástrico ou pancreático, podendo portanto ulcerar ou sangrar.
A maioria não comunica com o lúmen duodenal, pelo que os trânsitos não têm utilidade. Podem demonstrar estenose luminal, por
efeito de massa extrínseco ou intramural.
TC: massa quística bem definida; hemorragia intra-quística pode ocorrer (espontaneamente hiperdensa).
A presença de nodulação mural ou vegetações intra-quísticas devem levantar a suspeita de malignização (extremamente raro).

A distrofia quística da parede duodenal é uma entidade relacionada com a pancreatite da goteira pancreato-duodenal, sendo agora considerada parte do mesmo espectro de pancreatite paraduodenal.
Corresponde a tecido pancreático heterotópico na parede duodenal; cuja obstrução ductal leva a processo inflamatório que resulta na formação de lesões quísticas intra-parietais.
Há 2 padrões principais: ++ padrão quístico >1cm (DD com divertículos intraluminais); -- padrão sólido < 1cm (DD com neoplasia duodenal).
Quistos encontrados na submucosa ou muscular da parede medial de D2.
TC: múltiplos quistos pequenos e alongados na parede duodenal, associado a espessamento parietal de D2, +/- estenose luminal.

periduodenal fat and a small amount of

Antigamente chamada pancreatite da goteira. Forma incomum de pancreatite crónica segmentar que atinge a goteira pancreato-
duodenal, nas imedicações da papila minor.
A presença de fibrose pode levar à estenose duodenal.
TC: material mal definido, com densidade de tecidos moles, entre a cabeça do pâncreas e D2, que pode mostrar realce tardio.
Pode haver espessamento parietal de D2, formações quísticas intra-parietais e estenose luminal.
DD imagiológico difícil para tumores duodenais ou periampulares. A presença de quistos aponta para pancreatite paraduodenal.
Às vezes podem coexistir as duas entidades, uma vez que a inflamação pode ser causa/consequência da neoplasia.

Anomalia congénita rara que ocorre pela não rotação do broto pancreático ventral com o duodeno, resultando em tecido pancreático
circundando D2.
Pode manifestar-se na 1ª década por estenose duodenal; altura em que o dx é difícil devido a ausência de gordura abdominal e
pequena espessura da banda pancreática.
Metade é diagnosticado em adultos; 66% incidentalomas; os restantes com dor abdominal, enfartamento pós-prandial, vómitos,
ulceração pós-bulbar, pancreatite ou obstrução biliar.
Trânsito: estenose excêntrica ou concêntrica, regular, da 2ªporção do duodeno.
TC: tecido pancreático completa ou incompletamente rodeando D2, podendo haver estenose luminal com dilatação a montante.
Contraste oral negativo e aquisição em fase parenquimatosa pancreática aumentam a acuidade.
CPRE ou CPRM demonstram a junção do canal da banda pancreática ao Wirsung ou Santorini.

DD para o GIST e leiomioma (devido à sua origem submucosa). Morfologia plana, contorno lobulado e crescimento endoluminal
apontam para pâncreas ectópico.
CPRM: pode demonstrar a presença de ductos dentro da massa, o que é patognomónico.

O termo aplica-se a um grande espectro de alterações congénitas, desde onfalocelo em crianças a não-rotação assintomática em adultos.
A posição da junção duodeno-jejunal e do cólon depende da fase em que falhou a rotação embriológica.
Na não-rotação, achado incidental em adultos, o ID situa-se predominantemente no lado direito e o cólon no lado esquerdo, sendo
menos provável o volvo.
Trânsito: Configuração em Z do duodeno. Junção duodeno-jejunal mais baixa que o bolbo e à direita da linha média.
TC: Duodeno não ultrapassa a linha média entre a aorta e os vasos MS. Por vezes, VMS está à esquerda da AMS. Cego em posição alta.

A destruição começa no duodeno e avança progressivamente para o íleo.
Maior risco de linfoma e carcinoma GI.
Gold standard para o diagnóstico é a biopsia duodenal.
Trânsito: duodenite, dilatação GI, floculação, pregas lisas, diminuição do número de pregas no jejuno e aumento do número de pregas
no íleo – padrão de pregas jejuno-ileal reverso.
TC: espessamento parietal, dilatação GI, diluição do contraste oral (excesso de secreções), invaginações não obstrutivas e transitórias,
adenopatias, engorgitamento vascular mesentérico.

O Lúpus é uma doença auto-imune multi-sistémica que induz vasculite necrotizante.
Quando atinge o trato GI é geralmente multifocal, transitória e migratória, podendo atingir qualquer parte do intestino (++delgado).
Vasculite dos pequenos vasos mesentéricos -> isquémia; hemorragia, ileus, ulceração, perfuração.
Trânsito: “thumbprinting”, espessamento de pregas e separação de ansas de delgado.
TC: isquémia mesentérica em doentes jovens, em locais pouco habituais, afetando tanto o ID como IG; associado a doença genito-
urinária (nefrite lúpica, cistite, hidronefrose). “Skip lesions” (≠ isquémia mesentérica por tromboembolia – padrão contínuo).
Manifestações radiológicas semelhantes para todas as vasculites. História clínica para DD.

Consequente a colecistite subaguda ou crónica, com erosão da parede vesicular, formação de fístula colecisto-entérica (++
colecisto-duodenal) e passagem de cálculos vesiculares para o trato GI, onde se dá obstrução intestinal.
O local da obstrução é geralmente no íleo terminal (local com menor calibre do intestino).
Radiografia: Tríade de Rigler - aerobilia, dilatação de ansas de delgado; litíase calcificada em localização ectópica.
Como apenas uma minoria dos cálculos vesiculares é radiopaca, nem sempre as 3 características são vistas no Rx.
TC é a modalidade de escolha nesta situação, ao detector a localização exacta do cálculo e da fistula colecisto-entérica.

Trânsito: defeito de repleção endoluminal que causa obstrução parcial ou completa do “oulet” gástrico, com dilatação gástrica a
montante.
TC: Mostra com melhor detalhe o cálculo, a fistula colecistoduodenal e a dilatação gástrica.

Nos adultos é encontrado um ponto de partida em 90% das causas de invaginação (≠ das crianças), sendo neoplásico em 65%.
Invaginação gástrica para o duodeno ocorre devido a um tumor móvel (adenoma, leiomioma, GIST, lipoma, hamartoma,
pseudopólipo inflamatório, adenocarcinoma).
Trânsito: encurtamento e estenose do antro, convergência das pregas mucosas gástricas para o duodeno; colar pré-pilórico;
alargamento do calibre duodenal e defeito de repleção correspondendo ao tumor propriamente dito.
TC: Demonstra e caracteriza a causa da invaginação; sinal do alvo (duplicação das camadas parietais formando anéis concêntricos),
quando em corte axial ao lúmen; imagem de tecidos moles em forma de salsicha, quando longitudinal ao lumen.

Trauma duodenal resulta de lesões penetrantes (perfuração) ou contusas (contusão ou transecção).
Como a maior parte do duodeno é retroperitoneal, não há sinais de irritação peritoneal.
TC decide o tratamento: se contusão -> tratamento medico; perfuração -> tratamento cirúrgico.
Trânsito: A hemorragia intramural manifesta-se como espessamento regular das pregas, em forma de pilha de moedas.
TC: Na lesão aguda pode ver-se o sinal do alvo – edema da submucosa, e realce da mucosa e muscular. • Hematoma duodenal manifesta-se por espessamento parietal de atenuação variável conforme o “timing” do trauma. • Deve-se suspeitar de perfuração quando há uma extravasamento de contraste oral para o retroperitoneu, gás extra-luminal ou
solução de continuidade na parede duodenal. • Hematoma ou líquido retroperitoneal, densificação da gordura e transecção pancreática podem ocorrer tanto na contusão como
perfuração duodenal.

Situs solitus (normal situs) is inferred when the aortic arch, cardiac apex, and stomach bubble are located on the left. When these organs are on the right, situs inversus is
present. So, situs inversus refers to an anatomic arrangement that is the mirror image of situs solitus.
It is found in 0.01% of the general population.
20% of patients with situs inversus also have Kartagener Syndrome (which includes male sterility, bronchiectasis and sinusitis).
CT - mirror-image anatomy of the viscera. The heart and great vessels are a mirror image of their normal anatomy; the left hemithorax contains a lung with 3 lobes,
whereas the right hemithorax contains a lung with 2 lobes; the liver and gallbladder are on the left side, whereas the spleen and stomach are on the right side. The
remaining internal structures, including the duodenum, are also a mirror image of the normal.
UGI series – The 1st and 2nd parts of the duodenum are on the left of the midline, while the 4th part is on the right.
When there are no associated diseases, the only importance of recognizing the presence of this condition is due to the fact that an eventual surgical procedure can be
done with an adequate surgical incision.

Duodenal diseases may produce radiographic
changes in the thorax and abdomen.
In fact, sometimes an abnormal radiograph may
be the initial clue to the diagnosis of several
illnesses of the duodenum.
