Epidemiologia. Representan: 10 – 15% de la CC. Son los que con mas
frecuencia aparecen de forma aislada en jóvenes y adultos.
CIA – OS: (70%) 5-6 casos por 10000 nacidos vivos) y con una
frecuencia 2/1 en mujeres.
CIA – Seno venoso: 10%. Prácticamente siempre con drenaje
venoso pulmonar anómalo parcial a vena cava superior o a
aurícula derecho asociado.
CIA – OP: 20%. Caudal a la fosa oval.
CIA – Seno coronario: menos del 1%, habitualmente con VCS izq.

Anatomía patológica. No toda CIA corresponde a un defecto septal.
El verdadero septo auricular está constituido por la valva del
foramen oval y la parte inferior del borde muscular.
El sector postero-superior: Septum secundum.
El sector antero-superior: Raíz aortica.
Septo membranoso + trígono fibroso derecho + tendón de
todaro: Cuerpo fibroso central.

CIA tipo ostium primum. Este tipo de comunicación es un defecto del septo
atrioventricular (DSAV) con shunt limitado a las aurículas.
Comparte con el conjunto de los DSAV diferentes aspectos:
todos tienen una aorta no acuñada entre los orificios
auriculoventriculares, una unión auriculoventricular común, una
desproporción entre las dimensiones del tracto de entrada y el
de salida del ventrículo izquierdo y, además, un componente
izquierdo de la válvula aurículoventricular que tiene tres valvas.

Fisiopatología. Genera shunt I – D, con sobrecarga de volumen en cavidades
derechas e hiperflujo pulmonar. (Tamaño, gradiente de presión, compliance
ventricular, resistencias vasculares pulmonares y sistémicas, valvulopatia mitral adquirida o
enfermedad coronaria) 1
Shunt I – D: Reducción del alargamiento longitudinal de fibras
musculares AD en la eyección ventricular. Rigidez de la pared AD.
Pasaje: Telesistole y protodiastole ventricular y refuerzo final de la
sistole auricular. Sobrecarga volumen AD y VD. Hiperflujo pulmonar.

Fisiopatología.
HTAP: Agresión endotelial. Fragmentación de la barrera
subendotelial y liberación de GF de las células musculares lisas.
Proliferación de la intima. 1
Remodelación AI y FA. 2
Cianosis, poliglobulia, hipoxemia y disfunción plaquetaria (Ictus
cerebral y pulmonar).
PO: Arritmias auriculares: Taquicardias o FA. (Foco ectópico en
relación a cicatriz quirúrgica)

Cuadro clínico. Los pacientes portadores de CIA mayores de 15 años de edad
pueden cursar asintomáticos o comenzar (50% a partir de los 20
años) a manifestar diferentes signos y síntomas. (Infecciones
respiratorias reiteradas, fatigabilidad, disnea de esfuerzo, arritmias auriculares,
HTAP, enfermedad vascular pulmonar, hemoptisis, insuficiencia cardíaca derecha,
cianosis.)
A partir de los 50 años de edad 100% presentan sintomatología
significativa.
Los dos síntomas más comunes de presentación son disnea y
palpitaciones.

S1 normal o desdoblado con acentuación del cierre tricuspideo.
Soplo mesosistolico eyectivo en FP.
S2 desdoblado amplio y fijo. 1
Retumbo mesodiastolico tricuspideo. (Shunt severo)
Soplo de Insuficiencia. (HTAP en mayores de 40 a.)

En todos: PR prolongado. 1
CIA tipo OS: Eje QRS a la derecha, Hipertrofia VD (rSR’, rsR’ en
V1 y V2 con duración normal de QRS), BCRD o BIRD.
CIA tipo seno venoso: Desviación de eje de onda P a la izquierda
(P neg III).
CIA tipo OP: HARI asociado. 1
HTAP: onda P prominente en II + hipertrofia VD.

Cardiomegalia.
Dilatación de AD y VD. (Aumento del contacto esternal en el
perfil).
Dilatación del tronco de la arteria pulmonar y sus ramas con
aumento de la circulación pulmonar.
En caso de enfermedad vascular pulmonar (Eisenmenger):
Dilatación del árbol arterial pulmonar central con
hipovascularización periférica (imagen del “árbol de invierno”) y

Exámenes auxiliares. Ecocardiografia:
Demostrar CIA y variedad anatómica.
Repercusión hemodinámica: Dilatación, etc.
Descartar lesiones asociadas.
Retorno venoso pulmonar. ¿Anómalo?
CIA – OS menor de 0,5 cm2. ¿Cierre percutáneo?

A sinus venosus atrial septal defect

In the presence of an
intracardiac shunt, Qp/Qs
provides a means to quantify
the magnitude of shunting. In
this example from a patient

Exámenes auxiliares. RMN:
Puede ser de utilidad cuando los hallazgos del ecocardiograma
son dudosos. Se logra visualización directa del defecto septal y de
la llegada de las venas pulmonares. Además permite cuantificar el
volumen del ventrículo derecho y su función.

Exámenes auxiliares. Cateterismo cardiaco:
Valor exacto PAP, (¿mas de 35 mmHg?) Evalúa la reactividad
vascular a la admistracion de O2 al 100%. (NO, Iloprost, etc.)
Medida exacta de resistencias arteriolares pulmonares.
Magnitud del Shunt.
Valorar sitios de salto oximetrico.
Medir saturación arterial de oxigeno.
Evaluar el retorno venoso pulmonar. (En CIA tipo seno venoso)
En mayores de 40, lesiones asociadas.

Tratamiento. Cirugía ofrece mejores resultados que tratamiento medico solo.

El tratamiento debe iniciarse lo antes posible. 2
El implante percutáneo es superior al cierre quirúrgico en las
complicaciones, aunque esta diferencia no es significativa en
menores de 40 años. 1, 2 y 3.

Indicaciones de cierre. Indicaciones: En uno o mas casos.
Dilatación de cavidades derechas.
Qp/Qs mayor de 1.5/1.
Embolia paradojal.
HTAP leve a moderada y progresiva.
RVPulm menor a 2/3 partes de la RVSist. O test (+) a vasodilatadores.
CIA pequeño asociado a arritmia, embolia o aneurisma.
Contraindicaciones:
Negativa del paciente.
Procesos infecciosos locales, locoregionales o sistémicos.
Síndrome de Einsenmenger.
Dx de CIA en la gestación.
Disfunción severa de VI.

Vía de cierre. ¿Percutáneo o Quirúrgico?)
El procedimiento percutáneo es eficaz en tratar defectos con una morfología
más simple, en cambio el cierre quirúrgico se realizará en defectos de
mayor tamaño y más complejos (con dos o más bordes deficientes) o
multifenestrados. 1
Aunque el cierre quirúrgico es seguro y eficaz, el cierre con dispositivo
implica menor morbilidad. 2

El cierre quirúrgico de los defectos cardíacos congénitos en adultos (incluida
la CIA) ha demostrado ser muy seguro, beneficioso, y de bajo riesgo. 4

31-year-old woman after insertion of 40-mm Amplatzer septal occluder (AGA Medical
Corporation), which is in good position.
Percutaneous closure of an atrial septal defect using an Amplatzer® device is demonstrated in
two patients. Such devices appear on echocardiography as echogenic structures within the area
of the atrial septum. A: Two devices (arrows) were needed to occlude two separate defects. Color
Doppler imaging can be used to detect residual shunting across the defects. LA, left atrium; LV,
left ventricle.
During device closure of an atrial septal defect, intracardiac echocardiography is often use to guide
deployment of the device. This series of echocardiograms demonstrates placement of an Amplatzer closure
device across a secundum atrial septal defect. After the left atrial (LA) device is positioned, the structure is
secured against the atrial septum before the right atrial component is engaged. Then, the deployment catheter
is released, allowing the device to straddle the septum and obscure the defect.

Cierre quirúrgico. Esternotomía media o submamaria derecha. Por atriotomia D.
Cierre con parche de pericardio autologo o heterologo.
CIA – OP: Valorar la posición anatómica del NAV. Plastia de la
valva mayor de la mitral. (ETE – TO)
CIA – Seno venoso: Redireccionar flujo pulmonar a AI.
Si HTAP moderada: Cierre con parche valvulado.
Si FA: Maze.

Cierre quirúrgico.
Pronostico: Evolución natural. Asintomático.
3ra década: Arritmias supraventriculares, disnea, intolerancia al
ejercicio.
20%: HTAP.
10%: Einsenmenger.
Riesgo de endocarditis.

Pronostico: Cirugía. Mortalidad a 8 años: 4%. (Arritmias, accidentes vasculares y
falla cardiaca)
La HTAP es un predictor de mala evolución.
Arritmias supraventriculares o defectos en la conducción AV que
requiere MCP-D.
En caso de CIA – OP, puede desarrollarse IM progresiva que
puede requerir de sustitución valvular.
