Exercício 1 INTRODUCTION: In the study of human biomechanics, it is often desirable to evaluate fatigue in the muscles

that are involved in performing a particular task. Identifying the phenomena responsible for this condition is a problem that in most cases is complex and requires appropriate research mechanisms. Isokinetic dynamometry (ID) and surface electromyography (SEMG) are two techniques widely used in studies on strength and muscle fatigue. Their effectiveness is conditioned upon a good understanding of their limitations and the adoption of procedures to fully exploit the potential of each one. The main goal of the present study is to verify whether the electromyographic parameters, especially the conduction velocity (CV), are sensitive to the fatigue instauration process within sets of maximal isokinetic contractions. CV is a basic physiological parameter directly related to muscle activity and still little explored in experiments combining ID and SEMG.

METHODS: Instrumentation architecture that combines ID and SEMG was used to estimate electromyographic and biomechanical parameters in protocols of maximum intensity isokinetic knee extension exercises. This architecture allows for limiting the parameter estimates to a specific region of isokinetic exercise, called the isokinetic load range (ILR), where one can consider that the angular velocity is constant and the SEMG signals are cyclo-stationary. Electromyographic signals were acquired using an array of electrodes.

CONCLUSION: The results suggest that CV and the other SEMG parameters, including amplitude and frequency descriptors, are sensitive to detect a fatigue process only in protocols that restrict the analysis to ILR and that also bring the subject to a state of fatigue quickly.

Exercício 2 Introduction: Individuals with mobility impairments associated with lower limb disabilities often face enormous challenges to

participate in routine activities and to move around various environments. For many, the use of wheelchairs is paramount to provide mobility and social inclusion. Nevertheless, they still face a number of challenges to properly function in our society. Among the many difficulties, one in particular stands out: navigating in complex internal environments (indoors). The main objective of this work is to propose an architecture based on Mobile Augmented Reality to support the development of indoor navigation systems dedicated to wheelchair users, that is also capable of recording CAD drawings of the buildings and dealing with accessibility issues for that population.

Methods : Overall, five main functional requirements are proposed: the ability to allow for indoor navigation by means of Mobile Augmented Reality techniques; the capacity to register and configure building CAD drawings and the position of fiducial markers, points of interest and obstacles to be avoided by the wheelchair user; the capacity to find the best route for wheelchair indoor navigation, taking stairs and other obstacles into account; allow for the visualization of virtual directional arrows in the smartphone displays; and incorporate touch or voice commands to interact with the application. The architecture is proposed as a combination of four layers: User interface; Control; Service; and Infrastructure. A proof-of-concept application was developed and tests were performed with disable volunteers operating manual and electric wheelchairs.

Results: The application was implemented in Java for the Android operational system. A local database was used to store the test building CAD drawings and the position of fiducial markers and points of interest. The Android Augmented Reality library was used to implement Augmented Reality and the Blender open source library handled the basis for implementing directional navigation arrows. OpenGL ES provided support for various graphics and mathematical transformations for embedded systems, such as smartphones. Experiments were performed in an academic building with various labs, classrooms and male and female bathrooms. Two disable volunteers using wheelchairs showed no difficulties to interact with the application, either by entering touch or voice commands, and to navigate within the testing environment with the help of the navigational arrows implemented by the augmented reality modules.

Conclusion : The novel features implemented in the proposed architecture, with special emphasis on the use of Mobile Augmented Reality and the ability to identify the best routes free of potential hazards for wheelchair users, were capable of providing significant benefits for wheelchair indoor navigation when compared to current techniques described in the literature.

Exercício 3 Introduction : The clinical survival of a dental implant is directly related to its biomechanical

behavior. Since short implants present lower bone/implant contact area, their design may be more critical to stress distribution to surrounding tissues. Photoelastic analysis is a biomechanical method that uses either simple qualitative results or complex calculations for the acquisition of quantitative data. In order to simplify data acquisition, we performed a pilot study to demonstrate the investigation of biomechanics via correlation of the findings of colorimetric photoelastic analysis (stress transition areas; STAs) of design details between two types of short dental implants under axial loads.

Methods : Implants were embedded in a soft photoelastic resin and axially loaded with 10 and 20 N of force. Implant design features were correlated with the STAs (mm2) of the colored fringes of colorimetric photoelastic analysis.

Results: Under a 10 N load, the surface area of the implants was directly related to STA, whereas under a 20 N load, the surface area and thread height were inversely related to STA.

Conclusion: A smaller external thread height seemed to improve the biomechanical performance of the short implants investigated.

OBJECTIVES:

The diagnosis of cirrhotic ascites is associated with significant morbidity, mortality, and reduced health-related quality of life. Adherence by health professionals to quality indicators (QIs) of care for ascites is low. We evaluated the effect of adherence to ascites QIs on clinical outcomes for patients hospitalized with new onset cirrhotic ascites.

METHODS:

The medical records of 302 patients admitted with new onset cirrhotic ascites were interrogated for demographic and clinical data and adherence to eight Delphi panel-derived QIs for ascites management. Associations between adherence to each QI and 30-day emergent readmission and 90-day mortality were analyzed.

RESULTS:

The majority of patients were males (68.9%) over 50 years of age (mean 57±12.83 years) with alcohol-related cirrhosis (59%). Twenty-nine percent were readmitted within 30 days. Patients who received an abdominal paracentesis within 30 days of ascites diagnosis (QI 1, relative risk (RR) 0.41, P=0.004) or during index hospitalization (QI 2, RR 0.57,P=0.006) were significantly less likely to experience a 30-day emergent readmission. Baseline serum bilirubin >2.5 mg/dl was associated with increased 30-day cirrhosis-related readmission (RR 1.51, P=0.03). A total of 18.5% of patients died within 90 days of index admission; median interval to death was 139 days (37–562 days). Pneumonia was the most frequent cause of death. Independent predictors of 90-day mortality included older age (odds ratio (OR) 1.03, P=0.03), increased Model for End-stage Liver Disease (MELD)-Na score (OR 1.06, P=0.05), primary SBP prophylaxis (QI 7, OR 2.30, P=0.04), and readmission within 30 days (OR 30.26, P<0.001). Discharge prescription of diuretics (QI 8, OR 0.28,P=0.01) was associated with reduced 90-day mortality.

CONCLUSIONS:

Early paracentesis in patients with new onset cirrhotic ascites lowers 30-day readmission rates, and early initiation of diuretic therapy lowers 90-day mortality.
